How to generate a feedback form
Generating a client feedback request takes around 60 seconds. Follow these steps:
- Enter the client or project name if you want it included in the output (optional).
- Choose the deliverable type — for example, website design, logo concept, video draft or copy draft.
- Select the revision round and feedback goal that match the stage of the project.
- Add a feedback deadline and any optional context or focus notes.
- Tick the feedback areas you want the client to review.
- Click Generate feedback form.
The feedback request will appear below the form, ready to copy or download.
Do I need an account?
No account is required to use ClientFeedbackForm.com. There is nothing to sign up for or install. Open the tool, fill in your details, generate and copy. The tool works entirely in your browser.
Does the tool use AI?
No. The tool does not use AI, OpenAI, machine learning or any external paid API. The feedback request is assembled entirely from preset templates based on your selections. All logic runs in your browser.
Where is my feedback form stored?
It is not stored by ClientFeedbackForm.com. The generated feedback request exists only in your browser during your session. When you close or refresh the page, it is gone.
If you copy or download the feedback request, that copy is stored on your own device and is managed entirely by you. The UGC Playbook LTD does not retain any record of what you generate.
Can clients submit feedback through this tool?
No. ClientFeedbackForm.com is a static generator. It creates a text-based feedback request that you copy or download and then send to your client using your own channel — email, a shared document, a message thread and so on.
The tool does not host a live form, does not have a client-facing interface and does not collect or receive responses.
How do I copy or download my feedback request?
After clicking Generate, your feedback request will appear in the output area. Two buttons are available below the output:
- Copy feedback request — copies the full text to your clipboard, ready to paste into an email or document.
- Download as text file — saves the feedback request as a .txt file to your device.
What happens when I click "Turn this into a Chasepad request"?
This button opens Chasepad, a related workflow product operated by The UGC Playbook LTD, with context indicating that you arrived from Client Feedback Form Generator. The deliverable type you selected is passed through as a parameter to help pre-fill the relevant workflow.
Chasepad is used when you want a live client workflow — a space where feedback, approvals, files, change requests, reminders and sign-off can all be tracked in one place rather than over email or messages.
Clicking through to Chasepad takes you to a separate product with its own Privacy Policy and Terms of Use.
Troubleshooting
If you run into an issue, try the following:
- Copy button not working: manually select all the text in the output area and copy it using your keyboard shortcut or right-click menu.
- Download not working: use the Copy button instead and paste the output into a text editor or document.
- Page not responding or form not generating: refresh the browser tab and try again.
- Nothing generating after clicking the button: check that JavaScript is enabled in your browser settings. The tool requires JavaScript to run.
- Display or layout issues: try using a modern, up-to-date browser such as Chrome, Firefox, Safari or Edge.
